An invoice needs enough detail for your client to pay it and for both sides to account for it later. These are the fields to include.

No. You can fill in an invoice and download the PDF without registering. An account is optional and only adds a saved history of your invoices, a paid or unpaid marker, and a logo that follows you between devices.
A valid invoice needs your business name and contact details, your client's name and address, a unique invoice number, the issue date, a description of the goods or services with amounts, the total due, and payment terms telling the client when and how to pay.
Enter your tax rate as a percentage in the totals section. The tax is calculated on the subtotal after any discount, which is the standard treatment in most jurisdictions, and shipping is added afterwards.
